The wellness personal health record system, or electronic medical record, is included as part of
a free suite of Web-services. You may upload any document - text or images - to their secure
personal health record server. If you are a previous corporate wellness program member,
any disease screening results you may have stored with them can also be accessed there.
You may give your physician special 'limited' access to your medical records so that they can
view and download documents and print them for their own files.
Also, you may authorize your personal physician to electronically and securely
forward any information in your medical chart directly to your personal health record.
Simply log in to your electronic medical record on the wellness website and send the special
records request cover sheet to your physician.
